Do restaurants have to provide allergen information?

Yes, but it's lighter than most people assume, and it varies by region.

UK: You must make information on the 14 major allergens available for every dish. Right now that can be given verbally, as long as there's clear signposting telling guests to ask. Owen's Law, currently under Food Standards Agency review, is campaigning to require it in writing on the menu.
EU: The same 14 allergens must be declared. For food served loose rather than prepacked, individual member states set how, and in many it can be provided on request rather than in writing.
Canada: Priority allergens must be labelled on packaged food, but there's no requirement to list them on restaurant menus, so it's usually handled by asking staff.

The pattern's the same everywhere: the information has to be available, but a verbal answer is currently enough in most places. Edible lets you put it in writing before guests even ask, which is the direction the rules are heading. The 14 allergens, explained.

What allergens do I need to list on my menu?

In the UK and EU there are 14 you must declare: celery, cereals containing gluten, crustaceans, eggs, fish, lupin, milk, molluscs, mustard, tree nuts, peanuts, sesame, soya, and sulphur dioxide. The FSA's best-practice guidance points the same way.
